Good Morning:  Please share this message with others.

Please see attached income guidelines for our Breast & Cervical Cancer Early Detection Program.  The program is for low-income, uninsured women ages 50 - 64 in the Greater Jacksonville area.  It provides clinical breast exam, Pap smear, and mammograms for women who qualify for the program. If a woman is diagnosed with breast or cervical cancer by the program, she is qualified to apply for Medicaid through us.

I also attached a copy of the flyer for the "Reach and Connect Project". The purpose of the project is to increase breast and cervical cancer screening rates through outreach and education to small groups of women, and to follow-up with those, seeking screenings in areas of high and moderately high late stage breast and cervical cancer. The long term goal of this project is to reduce breast and cervical cancer morbidity and mortality rates, health disparities, and health system barriers to quality cancer screening and care. For more information call (904)253-1610.

Thank you!
