Please see attached income guidelines for our Breast & Cervical Cancer Early Detection Program.  The program is for low-income, uninsured women ages 50 - 64 in the Greater Jacksonville area.  It provides clinical breast exam, Pap smear, and mammograms for women who qualify for the program. If a woman is diagnosed with breast or cervical cancer by the program, she is qualified to apply for Medicaid through us. 



I also attached a copy of the flyer for the "Reach and Connect Project". The purpose of the project is to increase breast and cervical cancer screening rates through outreach and education to small groups of women, and to follow-up with those, seeking screenings in areas of high and moderately high late stage breast and cervical cancer. The long term goal of this project is to reduce breast and cervical cancer morbidity and mortality rates, health disparities, and health system barriers to quality cancer screening and care. The presentation at this meeting was: Carrie Davis, Wealth Watchers Community

Development Corporation (CDC) Inc.

Specific concerns discussed at this CPAC meeting included:

 How can you have two neighborhood organizations with the same boundaries

and the same CDC only support one of the organizations?

 When does the community/neighborhood have a voice in what projects the CDC

handle?

 How does CDC's get paid?

 Sherwood Forest was part of the "Paint the Town Program" the City of

Jacksonville did not see the process from beginning to end?

 The following railroad crossings need to be repaired Cahoon Road North, two

blocks off of Beaver street; Highway 100 and 100A coming out of Starke, FL and

on Commonwealth Ave.

The following item was discussed or voted upon: The Stage Aurora Theatrical

Company requested a letter of support from the CPAC for a $500,000 grant. The CPAC

did not approve there request.

Other discussion: Soutel Estates have the follow issues in their community: steel

drainage pipes have fallen; ditches need to be cleaned; zero maintenance has been

conducted on the retention pond; the fence between the community and the city park

needs to be replaced and the traffic right away needs maintenance at the entrance to

the sub-division.

Affairs, Urban Core Parks, Membership and Public Safety).

Each subcommittee worked towards the following goals:

Specific concerns discussed at this CPAC meeting include:

• People receiving used bicycles from charitable sources which would require research into

the bicycle give-away programs in Jacksonville.

• Adequacy of street lights, sidewalk conditions, and an ordinance amendment prohibiting

bicyclists from riding on sidewalks in the city core.

• The Berkman Plaza Condominium has twenty townhomes without power.

• Operation Urban Blight Systemic Inspections has started and are being conducted.

• Park hours posted and enforced in Confederate Park.

Motions:

The Parks subcommittee made a motion to the CPAC that a written request to Parks and

Recreation be submitted capital improvement funds, to include a playground for Julius Guinyard

Park and metal trash cans for Henry J. Klutho Park. All agreed.

• The Westside was be negatively affected by the Planning Commission's approval of Limes Billiards

and a liquor store despite the CPAC's and neighborhoods' objections.

• Negative impact on Westside neighborhoods by Ability Housing apartments.

The following items were discussed or voted upon:

Approve Send letters to Mayor Brown and City Council in support of AquaJax

development.

Other discussion:

• JSO was thanked for the tremendous amount of work in Cedar Hills Estates to close down a house

of prostitution and a separate meth lab.

• JSO was thanked for their work in Grand Park.

• Municipal Code Compliance was thanked for their prompt assistance with two issues.

• JSO Zone 3 officer displayed very unprofessional behavior at the scene of a hit and run accident

near Deerwood. Chair Budd will assist the resident with making contact with the appropriate zone

commander.

• The Jacksonville Zoo was congratulated on the birth of a gorilla. A giraffe was also recently born.

• How can Governmental Affairs be improved? Staff support and the committee members' better

understanding of the process are needed.

• Chair Budd thanked Council Member Carter and Council Member Love for their reports and for

attending the CPAC meeting.

• The Cecil Behind the Scenes Bus Tour is May 28. Tickets are $20 and include a continental

breakfast, a tour of several Cecil sites, and lunch.

The presentation(s) at this meeting:

Ron Tittle, Public Information Officer, FDOT, presented on the major state roadway projects in the

Southeast District. The projects that Mr. Tittle covered were: the Buckman Bridge/I-95 Express

Lanes, the 9B Express Lanes, the 295 Express Lanes, the JTB/I-95 Intersection Improvement

Project, the University Blvd. / Beach Blvd. Intersection Improvement Project, the Overland Bridge

Project and the I-10/1-95 project. A number of the CPAC members expressed concerns about

the Express Lane Project; they are dissatisfied with the clear cutting of trees for the express lane

projects, they want more information about the sound walls that will be installed, they want tree

mitigation enforced and they want to be a part of the re-landscaping design process.

Specific concerns discussed at this CPAC meeting include:

The CPAC had a conversation about the impact that Charter School has on the DCPS,

redistricting, impact on property values, additional bussing and the impact on transportation.

The CPAC would like to speak to a State Representative on Charter School and how they are

accredited locally and state wide.

The CPAC discussed their concerns of the FDOT roadway projects that were mentioned above.

In December, the CPAC decided to postpone the 2014 Volunteer Recognition Awards for the

Chair, Vice Chair, and Subcommittee Chairs until February. At this meeting the certificates were

presented and the Chair, Vice Chair and Staff Rosemary Wesolowski provided refreshments for

all in attendance.

A number of political candidates were in attendance, each candidate was allowed to state their

name and announce which office they were running for.

The LUZ Subcommittee met at 5:45 pm and a large number of applications were reviewed. The

CPAC did not make any motions on the LUZ applications.

The next meeting of the Southeast District 3 CPAC will be held on:

Monday, March 23, 2015 at 6:30 p.m.

Specific concerns discussed at this CPAC meeting include:

 Landscape contractors on Airport Center Road discarded pine needles and other waste into street

drains. This issue will be reported to the appropriate authorities.

 A Northwest Jacksonville Economic Development Fund grant to Church's an established national chain

already doing business at the site, gives unfair competition to the small business owners in the area.

Nearby vacant properties could benefit from the grant.

 Neighbors near (2015-0023) the proposed Boney Road development will lose their Greenbelt status. If

the development is approved, JFRD will no longer allow controlled burns in planted adjacent pine

forests because of proximity of fires to homes.

 Over 200 residents, 85% from zip code 32226, met at the Oceanway Community Center and opposed

2015-0023, the proposed Boney Road development. Concerns include change in lifestyle, increased

traffic and increased pressure on existing schools. Infrastructure and schools are needed before

development is approved.

The following items were discussed or voted upon:

None Approve Letters to Mayor and City Council to request investigation regarding erosion of Raymond Drive properties

believed to be caused by Army Corps of Engineers' dredging.

None Approve Letters to Mayor and City Council to request ordinance to prevent use of portable basketball goals in or on

City Right of Way, to stop adults and children playing basketball in the City streets and to focus on public

safety.

2015-0015 Deny An ordinance appropriating $58,220 from the Northwest Jacksonville Economic Development Fund

(NWJEDF) to provide a grant to HDW PARTNERS II, INC. To fund the required off-site infrastructure

necessary for the development and construction of a new 1,850 square foot CHURCH'S CHICKEN

RESTAURANT to be located at 1855 DUNN AVENUE. (THE PROJECT), as initiated by B.T. 15-028;

providing a carryover of funds into fiscal year 2015-2016; approving and authorizing the mayor, or his

designee, and the corporation secretary to execute and deliver the redevelopment agreement between CITY

OF JACKSONVILLE and HDW PARTNERS II; waiving certain NWJEDF project criteria approved and

adopted by ordinance 2007-281-E; providing an effective date.

Reasons for denial: The grant would give unfair competition- other small businesses in the area do not have

the benefit of this grant. Churches is a national chain; the business is already open and running; vacant

properties nearby could benefit from the grant.

2015-0021 Deny An ordinance adopting a small scale amendment to THE FUTURE LAND USE MAP SERIES 2030

COMPREHENSIVE PLAN by changing THE FUTURE LAND USE DESIGNATION FROM LDR (LOW

DENSITY RESIDENTIAL) TO CGC (COMMUNITY/GENERAL COMMERCIAL) on approximately 3.68+ acres

of land located in COUNCIL DISTRICT 11 at 117 KATHERINE ROAD and 109 KATHERINE ROAD between

ELIZABETH LANE and OCEANWAY AVENUE, owned by GRADY E. BRADDOCK, SR and DEBORAH M.

HOWELL, as more particularly described herein, PURSUANT TO APPLICATION NUMBER 2014C-017;

providing a disclaimer that the amendment granted herein shall not be construed as an exemption from any

other applicable laws; providing an effective date.

Reasons for denial: In hearing from adjacent neighborhood, clarification of use is needed. The community has

concerns – the developer needs to speak to the community to resolve the difference. The CPAC's tradition is

to deny when neighborhoods have unresolved concerns.

2015-0022 Deny An ordinance rezoning approximately 8.18+ acres located in COUNCIL DISTRICT II on 109 and 117

KATHERINE ROAD and 12953 NORTH MAIN STREET between MAIN STREET and GILLESPIE AVENUE

(R.E.NO (S) . 107598-0000,AND 107605-0000). As described herein, owned by GRADY E. BRADDOCK, SR.

and DEBORAH M. HOWELL, from COMMERCIAL COMMUNITY/GENERAL-2 (CCG-2) and RESIDENTIAL

LOW DENSITY-60 (RLD-60) DIRTRICTS TO PLANNED UNIT DEVELOPMENT (PUD) DISTRICT, as defined

and classified under the zoning code, to permit storage of RVS, BOATS and TRAILERS, as described in the

approved written description and site plan for THE GATOR CITY STORAGE, LLC PUD, pursuant to FUTURE

LAND USE MAP SERIES (FLUMS) small –scale amendment application number 2014C-017; providing a

disclaimer that the rezoning granted herein shall NOT be construed as an exemption from any other

applicable laws; providing an effective date.

Reasons for denial: In hearing from adjacent neighborhood, clarification of use is needed. The community has

concerns – the developer needs to speak to the community to resolve the difference. The CPAC's tradition is

2015-0023 Deny An ordinance rezoning approximately 395+ acres located in COUNCIL DISTRICT II at 14250 BONEY ROAD

between GROVER ROAD and SPRING HAMMOCK ROAD (R.E NO (S) a portion of 159735-0000 and

159733-0030), as described herein owned by CITY NATIONAL BANK OF FLORIDA, TRUSTEE, from

RESIDENTIAL RURAL-ACRE (RR-ACRE) DISTRICT TO PLANNED UNIT DEVELOPMENT (PUD)

DISTRICT, as defined and classified under the zoning code, to permit the cluster development of a single

family community as described in the approved written description and site plan for the EDWARDS CREEK

PRESERVE PUD; providing a disclaimer that the rezoning granted herein shall NOT be construed as an

exemption from any other applicable laws; providing an effective date.

Reasons for denial: Environmental issues- bordered on 3 sides by water - septic tanks and wells adjacent to

wetlands and waterways; traffic and safety issues on already congested roads, school capacity/children are

already attending in portable classrooms - middle school is above capacity. Interior lots are 50' – lots need to

be larger.

2015-0053 Approve An ordinance AMENDING CHAPTER III ( SPECIAL REVENUE AND TRUST ACCOUNTS), ORDINANCE

CODE; creating a new SECTION III.870 (LIBRARY FINES SPECIAL REVENUE TRUST FUND), ordinance

code , to create a trust fund to allow the revenue generated from the JACKSONVILLE PUBLIC LIBRARY from

the collection of fines to be retained by the library; providing an effective date.

2015-0065 Approve An ordinance rezoning approximately 10.10+ acres located in COUNCIL DISTRICT II at 3191

ARMSDALE ROAD between INTERSTATE 295 and ARMSDALE ROAD (R.E. NO(S). 019463-0100) as

described herein, owned by JACKSONVILLE TRANSPORTATION AUTHORITY from COMMERCIAL

COMMUNITY/GENERAL-1 (CCG-1), PLANNED UNIT DEVELOPMENT (PUD) and RESIDENTIAL RURAL-

ACRE (RR-ACRE) DISTRICTS TO PUBLIC BUILDING FACILITY-1 (PBF-1) DISTRICT as defined and

classified under the zoning code, providing a disclaimer that the rezoning granted herein shall NOT be

construed as an exemption from any other applicable laws; providing an effective date.

2015-0070 Approve An ordinance making findings; granting the application of JACKSONVILLE AIRPORT ("JIA") for the initial

certificate of PUBLIC CONVENIENCE AND NECESSITY (CON) for operation of JIA'S REGULATED

GARBAGE INCINERATOR FACILITY at JACKSONVILLE INTERNATIONAL AIRPORT, providing for

conditions of said certificate; incorporating exhibits; providing an effective date.

2015-0076 Approve An ordinance releasing THE CITY'S MINERAL RESERVATIONS on THREE PARCELS A,B AND C and

releasing THE UTILITY EASEMENT ON PARCEL A, said parcels being former RIGHT –OF-WAY in

DISTRICT II making up the traffic circle where TRADEPORT CIRCLE EAST met TRADEPORT

DRIVE, which parcels were abandoned by THE CITY ORDINANCE 2014-760-E, at the request of the

developer, THE HASKEW COMPANY ("APPLICANT") for the construction of improvements to the surface

estate and the creation of jobs attendant thereto; declaring THE MINERAL ESTATE and THE UTILITY

EASEMENT to be surplus to the needs of the city; waiving section 122.423, ordinance code, to authorize THE

MAYOR AND THE CORPORATION SECRETARY to disclaim THE MINERAL ESTATES to the surface

owners and quitclaim THE UTILITY EASEMENT for no additional consideration; providing an effective date.

Presentation(s):

David Roman, Mayor's Liaison, City of Jacksonville, presented on the various COJ Initiatives

and Programs. Spoke on the Renew Jax and the Renew Arlington project and the Renew

Jacksonville Community Empowerment Days. The Community Empowerment Day in Arlington

will be held on April 18, 2015 - Caroline Arms Apartments, 6457 Fort Caroline Rd. (City Council

District 1. The goal of the day is to bring service providers from throughout the community,

directly to the neighborhoods of the residents that need them. The goal is to strengthen families

and communities. Mr. Roman also mentioned that Parks Department is actively hiring for

summer positions at most parks and for a variety of different programs. Applicants must be 18

years of age, unless they are a certified lifeguard. Blue Cypress Community Center will also be

re-opened in the near future.

Specific concerns discussed at this CPAC meeting include:

• The CPAC members were interested in learning the details of the sign waiver ordinance

and how it applies to political signs.

• A number of the CPAC members stated that they have called 630-CITY to complain

about a tire store located at 6206 Merrill Road, 32211. The CPAC members were very

concerned about the appearance and the amount of tires that are stacked all over the

property.

The following motions were considered:

The Land Use and Zoning Subcommittee met at 5:00 p.m. The subcommittee reviewed several

applications and 4 motions were made:

Approval of Variance V-15-02 of the request to allow a reduction to the required width of the lot at 1671 St. Johns Bluff

Road for the following reasons: the site is already developed in a constrained configuration and relief is necessary

to utilize the existing structure. Redevelopment of the existing property is more efficient, historically respectful and

ecologically sustainable than the demolition of the existing structure and total rebuild.

Approval of AD-15-06 of the request to reduce requires landscape and parking requirements per application at 1671 St.

Johns Bluff Road.

Approval of Exception E-15-04 for the request to expand the existing 38 bed assisted living facility to 150 beds for the

following reasons: The existing use has not generated adverse impacts to the neighborhood. The use is consistent with

the surrounding residential uses and contributes a public benefit to the community.

Deny Rezoning 2015-064 Melissa Grove PUD and denied due to the following observations: The proposed use is not

consistent with the surrounding single family character of the area. The height of the proposed building is excessive

for the neighborhood. Parking spaces that block other parking spaces creates inconvenient and conflicting actions.

There are not adequate parking spaces for services vehicles and visitors. Traffic will have a significant adverse impact to

surrounding road system.
